Fayette County Memorial Hospital’s Medical and Surgical Associates recently received a top-tier designation from the National Rural Accountable Care Consortium. This makes the clinic among the select 15% of enrolled practices to earn the designation. Selection is based on improved quality performance on at least one health outcome measure, a high score on the Practice Assessment Tool and patient and family engagement efforts.

The program monitored metrics for annual wellness visits, controlling high blood pressure, tobacco use screening & cessation, colorectal cancer screening, influenza vaccinations and other outcomes. “We saw improvements in nearly all clinical outcomes,” said Dr. Emily Johnson, FCMH Rural Health Center Medical Director.

“We have been particularly successful at increasing the number of Medicare patients receiving their Annual Wellness Visit. This visit is a free visit designed to help prevent disease and disability based on your current health and risk factors. At this visit providers perform a health risk assessment that may include a review of medical history, height, weight, blood pressure, and body mass index measurements, screening for memory impairment and fall risk assessment. At the end of the visit the patient is provided with a personalized checklist for appropriate preventative services.”
